Please complete this application to be considered for any scholarships, except for Music and Visual Arts offered by Lake Catholic High School. Please provide a copy of your final 7th grade report card and 8th grade first quarter progress report in the link below.

Lake Catholic General Scholarship - Class of 2029

NOTE: Any scholarships awarded can only be used for tuition. If your tuition balance is less than the scholarship amount, the scholarship payout will be enough to bridge the gap and cannot exceed the balance.

On behalf of the Zappitelli family, we are pleased to announce that applications are now being accepted for the Mary Zappitelli Scholarship. This scholarship, valued at $1,000, was created in memory of Mrs. Mary Zappitelli, a beloved teacher at Lake Catholic for 17 years. Any Lake Catholic student is eligible to apply.

This year, two scholarships will be awarded. All Lake Catholic students are eligible to apply. The deadline to submit is November 1, 2024

Application for Mary Zappitelli Scholarship

Lake Catholic High School offers music scholarships for incoming freshmen who plan to be involved in vocal or instrumental music during their high school career. 

These scholarships are renewable for each school year while a part of the program - either band or choir. Scholarships are awarded to vocalists and instrumentalists based on audition ratings. 

Scholarship renewals will be assessed in the classroom each year. 

All applications are due by Friday, November 15 at 11:59pm.

Lake Catholic Music Scholarship Application

Qualifications for applicants for the 2025-2026 school year:

1. Applicant must be admitted to Lake Catholic High School as a freshman for the 2025-2026 school year. 
2. Applicant must agree to participate in instrumental or vocal music all year at Lake Catholic as a member of the performing ensembles listed below:

Band - Marching Band (after school rehearsals 2 days/week in the Fall, and performances), and Concert Band 1 class all school year (3 concerts)
Choir - Choral class all year (4 concerts) 

As part of this application, you are required to submit a video audition. Details are provided later in this application.

Lake Catholic High School is pleased to provide Visual Arts scholarships to the Class of 2029. 

To qualify, students must have an “A” average in their Junior High Art Class and enroll in any Visual Arts course during the 2025-2026 school year. 

To be renewed throughout the high school years, students must maintain a minimum of a “B” in all enrolled art courses. 

Completed applications including teacher recommendation and portfolio must be received by November 15th at 11:59pm.

Visual Arts Scholarship

Submit a Teacher's Recommendation from your current Art Teacher. Your teacher can submit the recommendation by emailing scholarships@fmakiosks.com. In the subject line, please enter “Visual Arts Teacher Recommendation” and your full name.

Submit a digital portfolio of your artwork to scholarships@fmakiosks.com. In the subject line, please enter “Visual Arts Portfolio” and your full name.

All artwork should be in one file.

If you did not complete the Class of 2029 general scholarship application, please submit your final 7th grade and 8th grade first quarter report cards to scholarships@fmakiosks.com. In the subject line, please enter “Visual Arts Report Cards” and your full name.

Please include 7 to 10 digital examples of recently created artwork.

Contain a variety of medium and subject matter including, but not limited, to:

• Painting: Tempera, Acrylic and/or watercolor
• 3D work: Ceramics, sculpture, etc. – include 2-3 photos from different angles/views
• Example of color, value, line, shape, form, texture and space
